Enrollment & Registration Forms

Enrollment & Registration forms are now embedded into E-Registration, our online registration system. 

The Enrollment/Registration process is explained on our Enroll Today page. Enrollment options for students who do not reside in Ashwaubenon, such as Open Enrollment, are explained on our Open Enrollment page.

Forms can be picked-up or dropped off at Cormier's office, 2280 S. Broadway. When registering, bring proof of Ashwaubenon residency such as your driver's license, utility bill or lease.
